Independent Investigation Uncovers Shocking Evidence of Medicare Hospice Fraud

Following shocking revelations of widespread theft of taxpayer dollars in Minnesota, Trump officials and independent journalists are now uncovering evidence pointing to a sprawling Medicare hospice fraud scheme in California, prompting yet another federal investigation. Just as the Minnesota Somali daycare scandal was a political death sentence for Governor Tim Walz, California’s fraud could prove disastrous for Governor Gavin Newsom’s presidential ambitions.

YouTuber and online influencer Nick Shirley is again at the center of the bombshell findings in California. At the end of 2025, Shirley gained nationwide fame after he went door to door visiting taxpayer-subsidized daycare providers in Minneapolis, asking for proof that the businesses were legitimate. At the daycares where someone even answered the door, most refused to confirm that there were any children there.

Shirley was not the first person to call out this potentially massive scandal, but his video investigation finally forced it into the national spotlight. For the first time, Democrat leaders in Minnesota faced tough questions on the scope of welfare fraud in their state.

Shirley’s latest investigation took him to California to look into hospice fraud, which the state’s Democrat attorney general has called an “epidemic.” Shirley specifically focused on a questionable strip mall in the Van Nuys neighborhood in Los Angeles, ground zero for the reported 1,500 percent explosion in new hospice agencies in the state. According to the California Globe, there are “500 hospice agencies crammed into a three-mile radius in Van Nuys.”

In a highly entertaining video, Shirley shows how hospice operators are raking in massive sums of taxpayer dollars without actually providing any services. In one instance, a motel had been converted into mini hospice offices, where Shirley confronts the alleged fraudsters. While some offices had a few workers in them – who did not want to answer basic questions – others were completely empty rooms.

As a local news outlet summarized, “Time and again, registered addresses turned out to be vacant shells: buildings with dead phone lines, piles of unopened mail, parking lots filled with luxury vehicles, and zero evidence of patients or caregiving, even as millions of taxpayer dollars continued to flow from public programs.”

As Shirley explains, fraudsters use the Medicare beneficiary numbers of seniors who are actually healthy to steal taxpayer funds. California hospice enrollment has inexplicably risen by more than 1,000 percent in recent years, leading to $170 million in suspect billings.

Critics – including Newsom – argue that Shirley is using sensationalist tactics or inflating the scale of the fraud. Just because a large number of supposed hospice providers rent low-cost space in the same area, they claim, that does not necessarily mean that they are all engaged in fraud.

But as Shirley’s Minnesota investigation proved, even if only a fraction of the explosion in hospice providers are fake, that still amounts to an astonishing amount of fraud and a major scandal. Moreover, his anecdotal reporting isn’t the only evidence that something is amiss in the California hospice space.

Just weeks ago, the Centers for Medicare & Medicaid Services, led by Dr. Mehmet Oz, sent its own team in to investigate the proliferation of hospice claims in California. “Eighteen percent of the whole country’s home health care billing is coming out of Los Angeles County,” Dr. Oz said recently, according to Fox News. “How is that possible?”

Further highlighting this extremely suspect spending is the incredible survival rate of LA County patients. The reality is that, while some hospice patients recover, the vast majority ultimately pass away shortly after entering hospice care. But Fox reported that, in “many [LA County] hospices, patients never die, with court records showing hospices billing the federal government for 18 months and more.”

But instead of working to root out the fraudsters who are stealing taxpayer dollars, Governor Newsom and California Democrats appear more interested in attacking the investigators. Newsom quickly accused Oz of racism for pointing out that a sizable chunk of the suspected fraud comes from “the Russian Armenian mafia.” (Oz, notably, is of Turkish descent. Turkey and Armenia have been at odds since the early 1900s, when Turkey killed 1.5 million Armenians in an act of genocide.)

Newsom also filed a civil rights complaint with the U.S. Department of Health and Human Services and said Oz “spewed baseless and racially charged allegations.” His press office then mocked Nick Shirley for going to daycares in California and asking questions about potential fraud as well.

But the great irony is that Newsom himself has claimed to care about stopping fraud. In January, Newsom boasted about removing 280 licenses from hospice operators in the past two years. CBS News later reported that “over 700 of the roughly 1,800 hospices in LA County trigger multiple red flags for fraud as defined by the state” and found that more than 40 percent of companies “still operate despite having multiple signs the state has outlined as indicators of fraud.”

All of this suggests that Newsom only wants to give the appearance of combating fraud without actually doing anything to stop it – hence why he’s so upset when journalists like Shirley and the federal government expose his failures and deception.

In March, Republican Assemblywoman Alexandra Macedo also challenged Newsom’s assertions that he is cleaning up the system. Like Shirley, she traveled to the controversial Van Nuys cluster and identified signs of potential fraud, such as a “dilapidated building” without a “wheelchair ramp” or “accessible parking.”

While Macedo challenged Newsom to do more, accountability is ultimately up to voters – and not just voters in California. As Newsom eyes the White House, taxpayers nationwide should ask themselves whether a governor who can’t (or won’t) police billions in federal fraud in his own state is deserving of higher office.

Just like USAID and Minnesota Daycares, I’ll lay 10-1 odds that Newsom is fighting this investigation because a large portion of this hospice fraud is being funneled back to the Democrat party.

I find nothing shocking about medicare/medicaid fraud. It’s rampant through out the system. Billions are lost almost daily while our govt. officials sit back and twiddle their thumbs. No accountability on any level. Thanks to this lax and lazy way, all of us pay dearly for every part of this system…..when costs could be reasonable, service fair and equitable, and care properly provided…..but no, we get the shaft while fraudsters rake in the dough, laughing all the way to the bank….and sadly our elected officials are probably joined at the hip with them somewhere along the way. Corruption with a capital “C”

Anyone watching the hen house? Criminals masquerading as day care and hospice services without oversight goes back to who’s allowing this fraud. Start there by removing them permanently and locking them up. Scammers are only the tip of the iceberg, what else is being stolen right under the guise government benevolence? But honestly, it’s always a democrat and typically a blue run state.

Exactly. The democrats are investigating it but never clean it up. Then when an outsider US citizen finds the fraud they scream foul. Trump is a dictator and Mr Shirley is wrong. How dare he look into this and find all this fraud. He doesn’t know what he found is true. Today at breakfast I found out that the sheep are planning another demonstration against Trump. To cover up that the dems in the Senate just funded the DHS with no changes but as is? After 39 days of shutting down the government and holding back the paychecks to TSA, Secret Service, Coast Guard and FEMA employees. Hurting all these employees not being able to pay rent nor pay for food and day care. Not to mention the inconvenience of millions of travelers with no TSA agents being available to make traveling by plane smooth and running on time. All this cost the country billions of dollars and put the flying public in danger of disaster and many workers at the airport go broke. But the sheep are going to protest tomorrow because Trump is bad. Schumer where are you now. Are you going to apologize to these workers at the airport and the millions of travelers. Are you going to return your salary and give that to the TSA agents? Are you going to do anything to help out these people for your grandstanding that didn’t have any affect nor changes on financing DHS. To keep our country safe we need DHS. A 5th grader knew that. Are you dumber than a 5th grader Senator Schumer?

Every blue run state and blue run city should be investigated for all kinds of fraudulent activities. I’m pretty sure they account for the highest percentage of fraud in America. Find them and shut them down. Put the savings in the Soc Sec trust fund. Then, people like me on social (in)security, wouldn’t have to worry about our checks getting cut by 24% in about six years. At age 70 I’m having to try to find ways to supplement my soc sec check in preparation for that cut. I have serious doubts that congress is ever going to get around to fixing the problem. They get their $174,000 a year paychecks and pensions when they retire but don’t do hardly anything to deserve that pay. Congress is broken and needs to be fixed.

I worked for Fed Ex at one point in Minnesota. The amount of packages going to Somali households absolutely falls in line with the Medicaid fraud. It wasn’t a box or two, it was 10;to 20 boxes on a monthly basis.

I wonder why most media outlets do not give this coverage. Guess calling them “media” is a bit of a stretch. Our MSM is too busy trying to find fault with our President, and gas-lighting their few viewers, that real news escapes their concern or report. Two years ago I reported over $500 of medical equipment on my husband’s Medicare statement that was not his. Was told they would check into it and his Medicare statement would show correction if billing error or fraud. To date nothing.

Thankfully, my Aunt was in a GOOD care facility! She was completely ‘home-bound’ lived to be 98, and was only admitted to Hospice in the facility when it was necessary. She did pass a few days after… I miss talking with her on the phone, because only until the last 6 months or so, her mind was still brilliant! She was always near her daughter in Fresno, where she could watch over her mother’s care.

Otherwise, how could scores of hospices, child cares, etc. have the SAME address but nobody in the bureaucracy catches it? Is there ANYWHERE where 10 of the same business share ONE address?

The trouble with most CaliforNIA citizens is that they are too busy eating granola, running on the beach, surfing, looking in the mirror and procrastinating to do anything about their state run by an idiot. Newsom’s own name might be on the hospice rolls and he doesn’t even know it. Medical corporations and large medical insurance corporations have had so many id breaches of their records it isn’t funny! Criminals are probably all driving around in huge black suv’s in every state of the US trying to “fit in” where they can set up computers and steal money all day long from hospice by filing false claims. It is very true that when hospice nurses are called in to a sick person’s life, that person dies quickly after! Isn’t hospice “end of life care?” So, how do the checks keep rolling out from the government faucets to pay 18 month and older hospice bills? And, are they checking ID and SS#’s especially to see if these numbers are being used over and over again? No because in order for Democrats themselves to steal taxpayer money on their special pet-agendas,they have to be sure money is pouring out of Washington as coverup for their own actions. Let’s start looking at some of those same Democrats that are screaming about looking into Hospice Care Funds…They might find more than the state of CaliforNIA committing these fraud schemes! What a world we have going on. So thankful that our president has allocated these findings because he has his hands full with the mess he was left by an idiot that SHOULD HAVE BEEN ON HOSPICE running the country with autopen!

Not surprised this is happening. Newsom’s focus is on the presidency and anything that makes him look worthwhile. This is a Democrat’s state which if possible I’d leave tomorrow. They rule CA and look at the messes we are road – Deficit spending, terrible roadways (here in Sac Co we have the worst roads in the entire state), low scores in our public schools, etc.

If you are riased in a fraud enviornent, you soon adopt many of the traits. Hospice is a valid need provider, and the fraud by certain groups using their name shou/must be investigated and if found, arrest, trial, and penalty is not enough. See how some of the groups only see American care, concern, and an altruistic attitude as a means to exploit. When discovered, take them into custody and hold them until trial and findings.

Those who are aware, allow, fail to act, and lead appropriately, to include them, for we need to get them all punished, and out of the Country as soon as appropriate. Those mentioned are criminals as well, so include them.

Americans – are you tired of being bilked, stolen from, victimized ?- Of Course you are.

THEN, stand tall, speak LOUD and demand enforcement and application of the criminal justice system NOW, and let’s jail those guilty and on release eject from the USA CJ system on their way out the USA and back to the place where from.

Next, those who allowed this are also guilty, then look at them as well.

Born and raised in the Van Nuys area. When I read that 500 hospices are located within a 3 mile radius in Van Nuys a great big smile popped out on my face and I muttered out loud ——– NO WAY!! There is a lot of fraud going on in that Van Nuys area. Crooks everywhere.

This young man’s interest, focus, attention and concern on this particular topic is sufficiently of interest to spark special attention to contacting the young man with personal experience information, in case any of the details are of use in the investigations. Been waiting and hoping for something like this to come along.
Any person who has personal involvement in the uncompesated in-home care of family, friends or loved ones suffering from the effects of increasing dementia, for example, who might have thought certain aspects of the subject’s having been placed into hospice care for extended periods (say, 18 months) by a primary physician or insurance provider, and then having been prescribed an extensive list of never-used medications by a visiting doctor who showed up once or twice during that period should be contributing their hospice histories with people who can investigate further. Fraud might not have been intentional, but over-prescribing at government expense should be addressed. People need to be polled to get evidence and material to turn over to Congress, if it would be appropriate amd useful.

Why have we still not heard what was done with the people who were arrested in the MN Somali fraud case? The news told us a couple of months ago that there were people arrested. Are they in jail? Nothing will change until these lying fraudsters are put in jail for their crimes! Just “outing” them does nothing. And those Somalis need to be deported. I hope Vance and his new hire will actually get these liars off the streets.
